What should my sponsor put there if I was born in the US and still live in the US for letter B? It doesn't really leave the "N/A" option.

It says "If the above address is not in the country of nationality of the person you are sponsoring...", so as far as I understand it you just skip (cross out in pen) b) outright.
 
And then fill in C with the obvious choice of proceed in the US?

In the December Applications thread, a couple had their application sent back for forgetting this question. One I would have overlooked myself! I missed the "IN Canada" part (as in, applying inland) in which case, like the question in fact states, I am applying outland.
