the medical procedure has changed recently, there are no forms now.....

only the passport, instruction letter and and IOM info sheet is sent back

it is also written on the instruction letter
1. Has this change expedited the medical process?
2. Have they increased the number of DMPs?

Procedure:

Loss of Gerrys CN Slip:

In case Gerrys original CN Slip/Receipt is lost, applicant will have to come personally along with the following documents

- An affidavit on stamp paper, duly attested by notary public/oath commissioner office, mentioning the loss of the CN slip
- A request statement to return the application/passport. Statement must also mention that if the lost CN Slip is found/retrieved somehow is, it will not be used to claim the collected application/passport rather will be returned to the Gerrys concerned office.

Thank God you haven't applied for Italy visa :p
For collection of applications of Italian Embassy, if Gerrys CN Slip is lost, in addition to the affidavit on stamp paper, the applicant will have to provide an FIR as well from the concerned police station.

My Father have dollar account for last 4months then how will we show the source of money as the money is in the dollars so source should also be dollars. please suggest????

It does not matter whether your account is in USD or GBP or PKR, all it maters is that your source of income, which in your case is in PKR, should justify the transaction of money in your account.

I am not sure about that since this is the first time i have ever been to an IOM for medical.

But inquired about how many days it takes for the results to reach chc and they said; 07 days (if this helps u compare)

They really need to add more DMPs to the list. In my medical, they mentioned that they are in the process of changing the medical procedure and will be adding more DMPs to the list. If they have done so then this would be good.

Any idea which bank to choose in Canada, specifically Montreal? Which does not ask for service charges from students?
