Yes, I have. Only 2 people including me have the packages signed by JOHN. Both on the same day. The issue is, our package was delivered to 47 Dorchester instead of 49. Hence the confusion.

47 is totally OK.
If you use the online tool to determine your eligibility, (and assumed you are eligible) you will find a simple step by step procedure. telling you how to file the application. Under that, they show the address below.

There are dozens of people whose application was received by Mike.
But still, only 2 people whose applications were received by JOHN..

Click on File>Download As.
It wont update automatically though.

There were two things at play here. If you read the history of the case, you'd be able to tell.
First, the package was delivered to 47 Dorchester Street instead of 49.
Second, It was signed by JOHN, a name not listed in spreadsheet before.
2 people, carrier=UPS, delivered same day
