question about the IMM0008ENU form..


for the country of residence question it asks for country, status, and the "from date" and the "to date"..

for the "from" date whch date do i enter for my spouse? he was born in india and has lived there all his life

and for the "to date"-- should i put the current date or put the words "present" because he still is a indian citizen

and also when I sign the documents ( i will be married in india and will be filing the paperwork when I get back)-- for my last name should i keep it as my unmarried last name or put my signature of my married last naame when signing the application package for spousal visa. because I plan on doing my last name change to my husbands when he comes and gets his canadian visa

Hi simkaur,

Are you filling the form on computer? If not, I would recommend you to do it. If you select Status = Citizen, then the from/to fields become grey and you cannot fill them. They are not needed.

Also, when you sign documents, use the name that appear in your passport. CIC will always refer to the passport.
