Application procedure if you have Provincial Nomination?

What is the application procedure/process for Express Entry if you have provincial nomination?
Please explain in detail.

If nomination was received before 1 January 2015, then you should send an old-style paper application package to Sydney and you cannot use your nomination in Express Entry.
Only special "Express Entry stream" nominations received in 2015 can be used in EE profile and province probably provided to you detailed description in the letter how to use your nomination - inside EE or not.
