The medical happens fairly early in the process. So IRCC is most likely still working to complete the review of the applicant, background and security checks, etc.
Sending the birth certificate won't speed up the process but you can certainly provide this to IRCC as further evidence of the relationship. Certainly doesn't harm to send.
