Appendix B - Updated Information Chart

Hi all, my wife and I are nearing the final stretch of our application process and I had a quick question. My wife is Swedish and I am Canadian. She lives in Sweden with our daughter who is also Canadian. We received a letter saying she is approved for PR and needs to submit an 'Appendix B - Updated Information Chart' with passport number, name on passport, passport expiry, eye colour etc. It also asks for this information about her spouse or partner(me) and her children.

Since I am am the sponsor and our child is not being sponsored(already a Canadian citizen) I am assuming they do not want the passport information of myself or our daughter, only that of my wife who is the principal applicant, correct?

Is there a form for this 'Appendix B - Updated Information Chart'? If there is, where can I get this form?

Or do I just type it out in a MS Word and print it out?

It's just an email when you get your passport request. I don't think it's officially on any website.
