Appendix A what should i write about Passport

Hi
I got passport request from Manila Embassy Canada they ask my passport and also send me appendex A And B.
In Appendix A they write this bellow:
Appendix A
Please complete the following information for YOU AND ALL YOUR FAMILY MEMBERS, whether accompanying or not.
We do not need the passports nor passport numbers of non-accompanying family members. Please write legibly.
I am a Applicant and my wife is spouse (Canadian) so should i give her passport no with me becasue i dont have any of other family member in my application.

So you wife is sponsoring you? Then you need to only submit your (applicant) passport.

Yeah i know but what about the form which they ask spouse name and passport no and my spouse is my sponser.

You need to fill on the paper about the info of your spouse sponsor...her family name,given name,date of birth,her passport number,date of it's expiry,height,eye color and marital status same as about your info.to what info they need on the form.
