Anyone living in UAE but applied in Islamabad office

I would like to enquire following regarding payment of RPRF:

1- Demand Draft to be made under which name?
2- Will Islamabad accept demand draft from UAE bank in CAD?
3- Along with RPRF, I have to pay processing fee for one of my new born dependent, which is given in Pakistan Rupees on Islamabad website. Can anyone please advise me the total amount of Bank Draft (including 2 RPRF and 1 processing fee for dependent under 22 years).
