Well they have requested all previous passport copies and all addresses since birth/ phone numbers since 2000. I wonder why :

they requested last month. I am giving it on paper but in tabular format just like in background declaration form

You should send them a CSE & ask about status of your file. It happen in my case as well, I got first "Add Doc Req" after inquiring about my application. Sometimes they need a wake up call.

Brother you have to write a CSE and ask them status of your file.
It will be a wake up call for them.

CSE means "Case Specific Enquiry". We need to fill a web form with required details & afterwards write our inquiry in message box. Visa Office reply back to our query within 28 days.

One of our SINP family stream applicant from India Gurdev Singh got his second AOR [response from concerned visa office] from NDVO after 81 days and then after 14 days he got medicals.
He was nominated on 24 oct. 2012.
So you are very near to receive medicals but i learned that LVO first request add. docs. like bank statements settlement plan, and updated forms. Then they request medicals. Did you provided settelment plan at any stage.
