So like many of you, my huband completed his medical exam in November 2012 and received the old form (Copy2).

I just got off the phone with CIC call centre and the guy told me that CIC will still accept the old forms if the doctor (who is an approved physician) has issued the old form.

So we should all be okay!!

Thank you so much for posting this! It is a great relief to know we won't be penalized for a shortcoming on the part of the Dr's who did November medicals and issued the old form.

Thanks for this good news, Spoken05! My wife did her exam on 12 December 2012, and we were worrying about the fact that the doctor's clinic gave her the old IMM 1017...

But now it's one less thing to worry about then!
