No, only the bio page and you can usually just scan it at home then email it as a 'reply' to their initial request. You certainly are way more patient then most I know. By now the email would've been gone within an hour of me getting it, if it were me! LOL Can always change PR cards and such at a later date because honestly, the ONLY time mine EVER sees the light of day is if we 'day trip' to the states to see the grandson or something LOL

****EDITED**** just to note that when I searched for information regarding this, I was informed via email and other various phone calls and such that being inside Canada I could not avail myslef of the consulate to do my passport and that I would need to send it and the required paperwork to PA to have it processed normally. I am trying to find that information online now and wondering if they have since changed the policy.

Found it. Follow the link here http://canada.usembassy.gov/passports/renewing-your-passport.html and look near the very bottom of the page and it notes that you should ONLY seek a passport from the consulate if you are currently under 16 years of age, if your passport was issued when you were 16 years of age or younger or there is an emergency. Otherwise, you are supposed to mail the forms to PA.

oh actually, if you go to the page for the specific embassy office, it will show the services. for ottawa, passport is one of them, and there is a way to make an appointment online. so perhaps it depends on the office?

haha! yeah, if i hadn't changed my mind about changing my name NOW, it would have already been mailed off. I seriously made this decision last week, and of course got the email on tuesday! I gave myself a few days to investigate my options and "think it over". open hours for the embassy was closed yesterday, so i really didn't have much choice there! :)
 
We never even thought of the whole name change dilemma and my wife has an appointment this Tues to do her med exam.. It's good to know that we can still process in her maiden name and change it later as doing it now would really delay getting started etc. Good info to know! :)
 
well i scanned everything and emailed it off today! i've decided to deal with changing my name after getting my PR card. wish me luck on both accounts! i'm still not getting my hopes up that this is truly the last step before COPR, even after looking at the ottawa spreadsheet [excited squeal]!!!

Now that it's a small possiblity my timeline has shortened, I got some stuff to do. namely... figuring out the whole car importing business! anyone know if I have to acutally go through all of that in order to land or if it can be done after official landing (assuming i'm using the car to travel at the time of landing)?

You can do it after landing for sure. I did. Just remember to declare it on your B4 goods to follow and you're good to go! :D

Yep, even if it's with you at landing, so far as I know. Just tell them you aren't quite ready with all the needed paperwork to import it and would like to legally import it later. They won't go out checking your car anyhow, or not likely they will. Just be careful with it in Canada after you have landed and such, make sure your US insurance and stuff will cover you and what not. Oh, and don't cross the border TOO many times driving it before you import it otherwise Canada will give you a 'stern' talking to EVERY time. LOL

My wife just had her med exam done using her maiden name.. If she were to change her name now would it mess all that up? We haven't submitted our application yet just the med exam has been done, and we could have it all proper on the application but then they would see the two last names.

No, it won't hurt anything to do with her medical exams as you will have to include the e receipt with the file anyhow. They will know what file to look for.

Thank you very much and sorry for the late response. Been working crazy long hours and in between trying to get everything else done. I sent our marriage certificate off in the mail yesterday, so when my wife receives it she will be starting the name change process so that will be all done before we mail in our forms :)
