Agent said not to update the application for Visa, what to do?

Okay so what we did was.

In IRCC we updated with the reason, my passport copy and us denial scanned copy. Not the application. let me know if I need to put the application too.
 
So let me know if only uploading my denial reason was enough in IRCC or if I need to upload a fresh application too?

You need to correct this error immediately and fire your agent. It's pretty much guaranteed your application is going to be refused and you are also risking a 5 year ban for misrepresentation. If you end up being refused and/or banned - there will be nothing to appeal because you will have been correctly refused/banned.

Again, correct this error immediately (even if you have to withdraw your application).

Please update it as soon as possible via case speciific inquiry. YOu can also send inquiry your self.
 
I got visa refusal second time for a same reason that my agent not disclosed visa denial entry to Canada.
If i re-apply visa for third time what are the chances?
PS: First time refusal reason is missing academic records, mark sheets.
