After ITA: Mistakes to avoid while submitting the app?

If you are application has been rejected or you were requested for additional documentation after submitting the application, could you guys please explain what were the mistakes.

This will help other candidates that are planning to submit the application soon. We highly appreciate your inputs.

So far, I am aware of these:
1. Issue: Submitting incorrect PCC Solution: Verify the latest info from CIC and read carefully
2. Issue: Submitting only translated docs and not originals Solution: Submit both translation AND original docs


Additional tips:
1. If possible, submit IELTS report and ECA evaluation



I will keep updating this list as you guys suggest any other lessons you learned the hard way.

Heard of these 2, obviously it is better to submit pdfs in addition to reference numbers-

3. Not submitting IELTS results report card
4. Not submitting ECA report

When you do submit an ECA report, also submit the degree for which you got the report.

I would love to know what was exactly the problem with the format of that job offer.
