Affidavit for Job Responsibilities..... Format required

Hi All,

Could any body, please send the Format of Affidavit for Job Responsibilities for Previous employer.

this is not possible to get the letter from previous employer for job responsibilities,
so is that good if i will provide the Affidavit for Job responsibility?
