I live with my CL for two years (we are together for 3 years) .... But the only name in lease contract is my name ... The landlord has no problem to confirm that we live there together as common-law ...

Do you have any sample/template of affidavit I should send her?

Affidavit has to be done before a notary.

Affidavit has to be done before a notary.

notary only can say the person who signed the affidavit is you. It does not authenticate the truth. so Notary is not at all required. I can get notary saying my DOB is xx yy zz. Notary only proves I am the person who got the affidavit done. it is my responsibility to prove the DOB is correct. so OP requires more proof.

Yes, but considering that owner will signed that with our names and together with lease contract ... one can say, that it is truth (that we live together) Don't you think? Or am I missing something?
