additonal family information?

there is a question in section A. spouse name is asked. I wrote down my husband's name here but I also have an ex-spouse. So after reading note1 I decided to put my husbands name but I am concerned if I should sign the section A or if I leave it blank. Cause there is an explanation and it says; if no spouse is listed sign below.

should I sign or leave it blank?

Leave it blank.you will see another section that asks about your ex.But in the family info include your now spouses name the ex goes on nothing but the space provided for the ex.You will need to attach a court divorce document and make sure it is the one that says final.
