Additional space

I was just wondering how to title the pages that I need for additional information. Do I just write the question number and the form or do I have to add more then that?

We wrote question number and the question itself, before that: form number, and sponsor's or aplicant's name

Yes remember and put sponsor and applicants name on the additional pages. CIC are notorious for losing things so its easier to match things up if there is a name on it.
