Additional space required in additional family info form

I am not able to put my detailed address in the form, so need more space.

Ques: Shall I use a blank sheet or a copy of this form? Please provide a bit detail of how to use either.

We used a copy of the form if we needed extra space. You can use a blank sheet just put the form info at the top and then the question number you are answering
