Additional Family Information - Form IMM5406 related question

Hi,
In the form IMM 5406, it is mentioned that the names should be written in English and in the native language. However, the guide doesn't seem to mention anything about the names in the native language:

1. Is it necessary to mention name in one's native language in that form?
2. If yes, then it should be placed in the box just below its English version... right?

Please advise.
