Do I need to fill out this form if I have no family members coming with me.
I have no dependents and my spouse is my sponsor?

Yes everyone needs to fill out this form. It also asks about your parents and siblings although they are not coming with you.

I would put "N/A" in any blanks that don't apply to you, but don't forgot to read the text by the signature blanks carefully and sign as needed. Some signatures are asking the person to confirm that he/she has no children, etc.
