Additional document request - kit, Application forms

Applied for PR-FSW thru express entry and I got a passport request yesterday.
With this I was requested to upload "kit, application forms".
I have no idea what is this form.
Does anyone know this?
Currently I am working in Canada with work permit status and I have less than 1yr experience. I was invited to apply under FSW in Express entry.

I got a request for additional document imm5669e background declaration to sustantiate gap in work experience. Form is available online in pdf but they have additional skew for signature and signature section in this form doesn't have provision for entering name manually... can I just fill details take print out, scan and upload it? Plz plz advise!! Thanks in advance!

I did not upload anything. I raised a case specific enquiry on this and did not hear anything yet.
Meanwhile, I was granted PR.

I was worried about this and I was given one day to upload. Never figured it out what is this document. I even called CIC and I was told to ignore this. Never got reply for CSE from CIC. However, PR application was approved. Was this requested with passport request? Because, I got it the same way. You may call CIC and I am 99% sure this should be a bug.

Thanks pal, it was with RPRF. I too have until today to upload it. did you add anything in or just leave it blank? Im sure it is a bug too. I can not find the form anywhere, just a link to a CIC webpage.

I didn't upload it. It still shows "We requested documents" in my CIC account. But my status shows approved and closed.

A new file is created called kit application form in my document section
