additional dependent/declaration form

Hello Everyone, I am applying under FSWP program and I m not living in canada and my wife will be accompanying me, so do I need to fill additional dependent/declaration form? what should I write in dependent type?

You don't need to fill it if it's only you and your wife. It's only required if you have more than 5 family members.
