Is there a form to fill out online and pay online for the access to information and personal information request form? My husband and I are together at this point so I can fill out the paperwork no problem, we just have no access to mail.
Also should I apply for access to information act or privacy act?
I just want to know if they have recieved our re medical yet, we cannot easily contact the doctor here (working hours and we don't speak the native language here well enough to ask about this over the phone!)
