ABOU WORK PERMIT FORM IMM5710 ....NAME PROBLEM

HI FRIENDS,

I am studying in canada and for my co op work permit i need to fill this form
but i am unable to validate it.... my full name is Amardeep Singh (as on my passport... but there
are two columns under full name....... family name and given name... in given name i filled my name
Amardeep Singh.... what about the family name... without it i'm unable to validate this form... please
anyone help me out to fix it........thanks in advance...a.d

Put Singh as the Family name and Amardeep as the given.

thanks for your quick response buddy..... but here is a student advisor in my college , she told me to put Amardeep singh in given name...
but she dnt know wht to put in family name... are you sure i can go with it... plz dnt mind... just asking
