5604e form and ex partner request

My ex partner moved to Canada 2 years ago and has asked me to sign the 5604e with a notary. He also asked me to complete a medical for my son. I am my son's main carer as my ex partner left the country with no warning and does not provide any financial support for his son. I live in the united kingdom.

Does my son have to live in Canada once this form is completed/ approved?

By signing what could be the possible implications should my ex decide he wants sole custody?

My ex says he put my sons details on his original application and by me not signing he cannot progress his own application. Is there no way that he could remove my son from his application?

My ex says he only wants to give my son the opportunity to come to Canada to live when he is older and its best to complete the application now or never? However what if he had some other reason.

Any information about this form would be greatly appreciated.

IMM5604 is permission from you for the child to immigrate. It is not required for your ex to immigrate, as he can include the child as non-accompanying. Unless you want your child to immigrate at this time, you should not sign that form.

The medical is required for the child to be included, so yes, you need to get it done. It does not grant permission for the child to immigrate but does leave the option open for future.

No I do not want my son to immigrate at this time. So I will do the medical but not the other form.
