5401 form question?

Getting ready to go to courier to mail application. I paid and printed receipt and completed receipt but it has a space for client number, I dont know what to put there and also is there an actual form 5401 or is that just my receipt?

5401 is the online payment, so yes, you print the receipt off to include with your application.

The client DOB is the Principal Applicant's.

The name and address should be that of whoever is paying.

The Client number can be left blank if you don't have a UCI yet.
