Yes. If you are still working in that organization and you are using these 5 months to gain points under CRS and/or program eligibility, you will have to provide evidence that you are still employed. For that purpose, you can use either a more recent reference letter or pay stubs, tax assessment forms, bank statement showing salary credits etc.

No. That isn't part of the standard documentation. In this case, standard documentation is the reference letter. But the OP has a 5 months old letter and they will need to provide supplementary evidence if they are claiming points for those 5 months as well. If they aren't claiming points for those 5 months, they don't need to provide any evidence.
