Just bring your original passport (have it Xerox), medical request from CEM which was emailed to you, 4 passport-size pictures and black ballpen.
You'll be disrobed during physical exam including underwear...hehehe...
Nice sharing, really appreciate your response jocomo. this is i think one of the best suggestion that i have heard so far. exclude the disrobing during physical exam,hahaha...
another question: what did you followed/submitted ? A or B ? my sponsor is a friend of mine, not related.

A. For Regular, Immigrant, Working, Student, Visitor Categories
Passport/ national ID (SSS, postal ID, voter's ID, PRC ID. driver's license, school ID) and birth certificate
Two (2) PHOTOCOPIES of passport/national ID and birth certificate (see format)
6 pieces recent passport photos (white or blue background)
Medical Instruction from the Embassy (Embassy Form 1017)

B. For Sponsorship Immigrant Category
