This letter is in reference to your application for permanent residence in Canada
WHY ARE YOU RECEIVING THIS LETTER?
We are in the final stages of processing and we need your passport(s) and the documents listed below in order to make a final decision. Please note, this does not guarantee that a visa will be issued.
WHAT ARE WE REQUESTING?
- Passports for yourself and each accompanying family member
Each person must have their own passport and there must be at least two blank pages in each passport
Ordinary passports are the only type of passport into which a permanent resident visa may be inserted. Diplomatic, official and special passports are not acceptable
If you have obtained recently a new passport, please send us a copy of the bio-page by e-mail so we can update the information on your file
If you change your passport after your visa has been issued, you will have to bring your old passport, visa and your new passport back to the visa office for re-issuance. There is a fee of 30,00 CDN for each visa re-issuance

Documents and passports must be submitted altogether with a copy of this letter. You have 60 (sixty) days to comply.
HOW TO SUBMIT YOUR PASSPORT(S) AND DOCUMENTS?
You or someone you have authorized must submit the passport(s) and documents in person to the visa office in Warsaw or in Ankara.

You can submit your passport(s) Monday to Thursday, from 08:30 to 11:30. You will have to come back to the Visa Office the next business day at 08:30 to get the results. We do not provide services on statutory holidays; please visit our website www.canada.pl for the complete list.

You can submit your passports Mondays and Wednesdays between 08:30 and 10:00. If you arrive late, you will have to come back the next available day for passport submission. You will have to come back to the Visa Office the next business day at 14:00 to get the results. We do not provide services on statutory holidays; please visit our website

WHAT ARE THE NEXT STEPS?
Your case will be reviewed one more time and if everything is in order, a final decision will be taken. You must come back to the embassy the following day to receive the results.
If a visa is issued, it will be valid until : _ March 7, 2013_________________________________
Please note that once your visa is issued, it cannot be extended. It will be your responsibility to travel to Canada before that date.
IMPORTANT NOTE:
If you authorize a third party to submit and collect your passports, please fill in the attached letter of authorization and submit it with your passports.
If there are any changes in your family configuration or your passport information, you must notify us before presenting yourself at the Visa Office.
