I used IMM5669E only personal history section in table form. Saved as pdf and sent to them. Also listed recent history first since that's what they asked to do. Also included travel history as well. Hope this helps.

Got a similar request last week. Within a day, they asked to redo the medicals again for myself and my wife. We did our medicals in Oct 2011. If your spouse did her medical in Jan 2012, they probably aren't going ask another one. Wait a couple more days.

Just found out that my application might be put on hold until my wife can take her chest x-ray. She's pregnant for about 22 weeks which means that she can't take her chest x-ray. The due date is IRONICALLY July 1st. What a day to have a birth. We've been waiting for PR for almost 2 years now...

Thanks!! They have an office in Calgary too so I could go there to get my wife's fingerprints. How did you send it to CIC? Did you mail it to them? Or L1 Solutions did it for you?

Got another request letter from Ottawa. This time they asked a set of original fingerprints for my wife. It didn't ask mine. I never submitted mine either. Weird. Anyway, does anyone know where to get a set of fingerprints in Canada? Do we have to go to RCMP to get one?
