Hello i received oath letter and prohibition form. Question i have on first page of prohibition letter Surname , given name , application number, group no, uci and date are blank. Should i write by myself or they supposed to be printed those information on the letter?

I applied october 2017 i passed test on july 2018 And status changed after 16 month

Hello My status change to decision made yesterday.when i click on that last update is my test date. Can i ask what does this mean? And how long usually does'it take to recieve ceremony date and why they didn't send when status changed to decision made.? Thank you
