great news , wish you good luck . yes i filled it (5490) , and you have to send it all using courier for the 1st time then you can use the web form , and they will clarify this issue . regards

they will mention in the letter to the VO , and you will received letter from VO with check list , this what happens to me and in my case Abu dahbi VO regards
