    Yes, I got the same request form 1017. Did my medicals at cdc, they took the form and just gave me receipt and proof of medical. Didn't have to uoad either of them. Finished medical on 7th and got them verified on 14th.i suggest you ask cdc if they have sent medicals to ircc first.
