Application fee is $150. Biometrics fee is $85. (biometrics requirement is depending your passport) You should only pay the medical fees at the recommended hospital. Where are you applying from?

I applied on 31 July. As I mentioned, I had a issue with the biometrics letter. Problem solved on 4th Sep. Since then no update.

If you are planning to submit the application offline then you can print an additional copy and write the names. I sent online application.
