Yung application ng mother in law ko was recieved by Canada Embassy-Manila last May 30, 2013, month of June passed by and we're on the the 8th day of July but til now.... no reply whatsoever from embassy.. ( sad but true...)

hello nats thank you sa reply, its ok not to include the use of representative form (IMM 5476E). its a personal preference. just like your case, my mother in law doesn't have an email and never had the interest to touch computers, I included the form for future use should the need arises...
