When the medical exam is requested, they can choose a panel physician anywhere in the world, even in a different country from where they live. However I recommend you use webform and inform IRCC about their correct mailing address unless you are representing them.

As long as you meet the minimum necessary income for last three consecutive years, shouldn't be a problem. Just keep in mind you need to add the number of people you have previously sponsored, to your family size.

You said IRCC asking for proof of upfront medical exam. That would be the proof. You can contact IRCC and double check with them. They may have asked it mistakenly. However I wouldn't ignore their request.
