So I called panel physician (emedical enabled) and this is what I found : if medical was already requested panel physician can do medical if you provide uci # or ime # or passport # and last name. Hope it helps .

you can fill one(Use of a Representative Form (IMM 5476)) now and submit via webform but you will need your parents signature on that.

not yet , but if we don't receive medical forms then we are going to get medicals done that way. But will need UCI's for mom dad and sister . I called CIC today they said they can provide me UCIs if I am the representative for each of them or the applicants can call to get UCI.

Yeah that's the problem , We can't create mycic account. our representative will have to do that but they don't create mycic accounts he said. :(

Hi MrsMN , I called panel physician they need UcI or IME number to send medicals, but only got my fathers UCI, which means they can only pull my fathers information with that but I need my mother and Sister medicals done as well.

I am still waiting for medical forms. Please let me know if creating mycic account helps . Thanks
