    What about for 10. Authority to Release Personal Information to a Designated Individual (IMM 5476) (NOT MANDATORY). I'm not including that form ,as it's not mandatory, should I leave the box on the checklist blank and write n/a or check it and write n/a or just leave totally blank. What did you do?

    I heard you can't write on already verified/barcoded pages but...?

    What about writing n/a on them? Is it allowed? Would it mess up the barcodes.     In the guide, it says to write "physically present" at marriage but no space!?

    This is in regards to the additional family members form. Did anyone write this? There's very little space to do so or did you just write married or attach a separate page?
