If you fail your exam, and you had an oath planned on the same day, then yes, you have to reschedule everything.

Looks like 1 is enough if you are already sending a passport. However, I remember someone on this forum getting their app sent back to them because they didn't have enough IDs. I am not sure if it ended up being the applicant's mistake or the agent's mistake.
