You can either print it out, filled it out, and bring to the test, hand it to interviewer when called. Or, you can just go to the test place and you will be handed over a same copy of prohibition form, fill it out, hand it to interviewer when called.

These is one possibility, that OP has to travel due to emergency family reason and is UNABLE to renew passport from origin country, this would qualify as urgent processing.

I have same day test as you. Mine is 11:45 and they told us that this is last test scheduled for the day as all the afternoon ones got cancelled. But there are some candidates for 1:30pm who came very early that the CIC guys let them took test with the 11:45 AM candidates. Lucky them.
