You need to get your credentials evaluated from an authorized credential assessment services organization (e.g. WES) to know exactly your education equivalence.

Yes fees is involved in getting your education credential assessment and for appearing in IELTS. For ECA you can check WES website and your local IELTS test providers.

Get the copies attested and sealed in an envelope from HEC and send to WES. Don't send original documents.

I sent my documents to WES the same day I got them sealed by HEC. Didn't face any issues. It usually takes 2 to 3 days in reaching WES office from Pakistan.
