Usually it will be "Place of Birth" mismatch with application. Try adding state name with commas or without commas with place of birth. Took me few days to figure this out and link the application.

In my case they processed remaining things. Because when I submitted my wife and baby medical report - I got passport request in 3 days. So it looked like all process is completed. YMMV

Yes. We took x-ray after pregnancy. CIC will finish all the background checks during that time. But once pregnancy is over CIC will ask you to submit baby passport and medicals and add them to the application.
