Try to get a letter from your manager/supervisor along with these documents. Also, add an LOE explaining your situation.

You got to mention all the addresses that you have stayed during the past 10 years with approx dates. Try to be as accurate as possible. However, if you miss something or put the dates wrong for a certain address, you don't have to worry about it as they understand its hard to 100% accurate. But...

Who is in your family? You can probably sponsor your immediate family members after getting your PR and moving to canada but that won't be under the express entry program. However, your spouse and kids are included on your express entry app and get the PR the same time as you.

It would be better if you can be as accurate as possible. It took me 3-4 hours and even researching on google maps of all the past addresses I lived at. They do run a background check on through various agencies so don't leave anything to chance. That would be my advice to you.

Try to get the english certificate from your city's authority. Its much better.

It would depend on your application. Time frame is variable. However, you can check the average processing time on the CIC website.

They don't ask for the date when you create your profile.

You cannot apply for just for yourself as you have a family and she will be considered in your application.

I got a request for Schedule A last year and I was able to submit it the very next day. I received my PPR within the next 10 days. So don't worry. I hope that you were accurate enough in your submission. I spent a lot of time on it making sure I was accurate and detailed. Best of Luck.

My passport might be lost/stolen with an unused PR visa? I have emailed my local visa office. What else should I do? Do they replace the visa in the new passport?
