There shouldn't be any problem which country bank account you are showing to provide proof of funds. In your case, since you are on H4 (dependent visa) it only makes sense that you have savings in India and not in the US since you are not allowed to work on H4 so no savings in US.

This is what I gather, if you travel through commercial transport (Air, Sea) then you will need PR card. If not, then you should have a PR Travel Document (which you can apply under urgent process and get it issued to you in fairly short period of time). But this has to be applied at the visa...

AFAIK, 1. You need to fill out the forms and take 2 copies of the form with you to submit and get a stamp on them at the time of landing. Please follow this helpful thread put up by Qorax to get all the relevant information regarding this subject...

That's correct, PCC is not issued for a particular time period. They just put a stamp in one of the pages in your passport indicating (along the lines) that your record is clean and there is no objection from indian consulate for you immigrating to canada.

Do you have the copy of the documents which the agency said that they have already submitted to the embassy? If you have that copy may be you can try uploading the same copy on CIC website and see if that will satisfy the requirement. Just an idea. Hope this helps.
