You have to include all dependents. You have a option of selecting accompanying or non accompanying. Other then taxes, there are no special consequences for US citizens.

For the COPR, Walgreens is good enough. For the PR cards, you should do a google search for "Canadian passport photo" and see if a professional comes up. Which city are you based in, maybe someone from that city can suggest a place.

Check the IRCC website for PCC process. BTW you need PCC from any countries your stayed over 6 months since 18 years of age. Not just the last 10 years AFAIK.

If it is original, you are fine. If you wish you could always include a letter of explanation stating that it is a scan of original which is black and white.

Must be based on Canada airports. In Toronto YYZ, we cleared custom in Canada before flying direct to Minneapolis.
