Guys, does this include all trips to and from country of origin or residence? the request says all your trips, while the form says excluding country of origin or residence please assist me as i have very few days left.

I guess yes, now the the question is, what if i dont want to do that anymore? cuz originally i thought it provides someone access as well as myself, but apparently all correspondences will be with that delegate only if you send this form.
