Yes，you do need to mention your work permit status in the form， there is a section asking you to fill in your temporary status in the past 5 years. If India is your origin country before PR，then you do not need to provide PC.

but I have never use the signature on my passport to sign anything in Canada，I used that signature in my home country， and in Canada I have only used my English name to sign all sorts of documents.
