    You don't fill anything except the social security card form. I would fill that out , print it and bring it with me.

    Depends. If you have really good credit score in the US, and have a good amount of money in your account, you can call your back and ask them to transfer your credit line to Canada. I did this with AMEX where they accepted to give me the same credit line in Canada.

    I received my passport yesterday from NYC office. They received the passport on Saturday and sent it back on Tuesday. So if anyone wants to change the VO to NYC, it takes about ~10 days to get the office changed and then just couple of days to send and get back the passport.
