Yes, he should go to VAC in lahore, they will send the pp to embassy in ISB. Only the members whose pp are being delivered need to fill in their info on appendix A. So, in your case, only your husband. For all the other rows i.e. spouse, child 1, 2, ... write "N/A".

I got an email... CIC will email you on whatever email address you provided on PR portion of the application i.e. on IMM0008 form. If you've got a representative, and s/he gave his/her email, then he will get the email. btw, is ur meds expired? did u get a remedical request yet?
